Project description:

Building a Healthier English-speaking community through the improved quality of life at all ages and stages

Adapt and pilot an existing health promotion program by establishing a partnership agreement with the CSSS, as identified through the community needs assessment that was undertaken in 2009-2010. Develop innovative information tools and animation strategies using local media and our website to promote activities. We will ensure community resources are utilized in the implementation of a health promotion program, through the recruitment and training of volunteers, in addition to working in collaboration with the school to break isolation experienced by seniors.

Project description:

Joining Hands for Health Care

The working group will develop a workplan identifying health promotion topics based on the community questionnaires as carried out in 2009-2010. We will partner with 2 CSSS territories to develop a services information pack for the ESC, in the area of Homecare.

CASA will pilot a frozen meals program for seniors in collaboration with the CSSS, and look towards solidifying a partnership agreement in delivery of the program for the senior population.

Project description:

Health Promotion Community Liaison

Vision will become the link between the CSSS health promotion and disease prevention programs and the English-Speaking Community of Gaspe, through the engagement of a Health Promotion Community Liaison worker who will build a strong and sustainable relationship with the public health partners ensuring access to health prevention activities and participation from the English-Speaking Community in health promotion planning processes.
