Project description:

Improving Health Promotion Programs for Seniors on the Lower North Shore

A community needs assessment will be undertaken to establish where seniors’ day centres and adapted transport are required. Coasters will work in partnership with the CSSS de la Basse-Côte-Nord to pilot a seniors’ day centre in a vulnerable community. A health determinant profile will be developed for the community that is piloting the day centre and adapted transport program, which will assist in establishing local health promotion and disease prevention priorities.

A community-sponsored health information campaign that directs the English-speaking community to appropriate health and social services will be developed with, and approved by the CSSS. Community resources will be enhanced through the development of a volunteer bank where volunteers will participate in delivering health promotion and disease prevention initiatives at the day centre.

The final component of the project will be the establishment of a community development model of best practice in implementing health promotion and disease prevention information in partnership with the public health system.

Project description:

Promoting Active Healthy Lifestyle Habits for Seniors on the North Shore

We will establish a community outreach program in each region resulting in an increased sense of cohesion within the English-speaking senior community and encourage them to maintain an active lifestyle. The project will provide seniors with greater and improved accessibility to information and activities; in addition it will help to identify services needed. We plan to conduct a needs assessment and inventory of seniors’ health and social services.

The project aims to promote healthy living and activities, promote participation and commitment of seniors in community life, combat isolation of seniors, improve the access to information, and the sharing of knowledge. We will develop an action plan and design seniors health promotion activities and create a monthly calendar promoting these activities to community members.
