Population Health Project

Population Health Fund Project Description:
"We Can Act: A Community Health Promotion Strategy
for English-speaking Communities"

This project promotes the participation of English-speaking communities in the development of public health strategies and strengthens their role in the Quebec’s health and social services system.

The project touches on health promotion strategies that engage different sectors and bring English-speaking communities directly into the development
of population health models in the newly created local services networks.

Project objectives are intended to strengthen collaboration within communities and with planning authorities and services providers.

CHSSN Community Public Health Strategy

To support English-speaking communities in the development of public health strategies, the CHSSN has developed a Community Public Health Strategy. This strategy encourages the English-speaking communities to become aware of public health programs offered in their regions; the building of relationships between community and public health professionals; and the developing and piloting of adapted public health programming. See below for a listing of  project descriptions per region.
